What you need to bring...

Welcome to Renaissance Orthopaedics! We are excited to have you become a part of our practice. We would like to make your first appointment go as smoothly as possible!

In anticipation of your scheduled office visit, please fill out a Patient Information Sheet (pdf). If you are being treated for a hip or knee problem we ask that you also complete either the Hip Evaluation or Knee Evaluation Questionnaire. For your convenience you may download the pdf files and print one for yourself or pick up a copy from our office. Please fill it out as best you can and bring it with you to your appointment (no need to mail it to us).

If you have had previous treatments for the problem we will be evaluating in our office, please bring with you any relevant medical information. This may include any or all of the following (Please bring only those records that pertain to the problem that you will be evaluated for in the office).

You must bring the following:

- Operative notes from previous surgeries

- Reports from previous MRIs or other imaging studies

Please remember to also bring:

- Your insurance card which will be scanned in to our system. Please contact your PCP for an insurance referral if applicable.

- Your copay which can be paid by cash, check, or money order and will be collected at the time of your visit

Should you find that you are unable to keep your appointment, please notify our office at least 24 hours in advance. We very much respect your time, and we try our hardest to stay on time!  We ask for your patience in advance and promise to make your first visit worth your while!

Our office is located at Magee Hospital.  We have a map and directions to our new office which is inside the main entrance, directly across from the grand piano in the lobby.   We also recommend that you take advantage of the valet parking provided by Magee.
